Clarification of the Activity Release Guidelines
After recent questions about the activity release guidelines, here is the clarification per Dr. Kennedy, in agreement with FISD and FHS guidelines: 

If you need to take your student home early from an event, i.e., at halftime during a football game, you must do the following:

    1. Pick up an Activity Release Form in the Band Hall or download it from www.fhsmbbc.org under Information then Forms. You MUST use this form.
       
    2. Fill out the form with the Student Name, Event, Date, time to be picked up, and signature of the parent picking up the student. A student can ONLY be picked up by his/her parent.
       
    3. Give the form to the head band director for his/her signature NO LATER THAN NOON the day before the event. The band director will take ALL the forms to the principal for his/her signature.
       
    4. At the event, the parent designated on the Release Form must sign out the student. In most cases, the head chaperone will have the sign-out sheet.
